Payment Terms:

I accept cash, check, most Employee Assistance Programs (EAPs) and Health Insurance Plans. As per the link below "Pay Online", I also offer clients the option of paying online through PayPal, which allows you to use your credit card as well.

HEALTH INSURANCE
I am IN NETWORK with the following health insurance plans: Aetna, Cigna, FiServ, Healthnet, ODS (most), PacificSource, Providence Open Options (most - other Providence see below), TriCare, Value Options, and several other plans that basically outsource their mental health services with these companies or in some other way are affiliated. The best way to confirm that your insurance plan will cover seeing me is to call them at the number on the back of your card and ask prior to our first session. Please also ask if there are any deductibles and what your copayment is per session, and if a pre-authorization is necessary for your plan type. I always encourage people to write down the first name of the person you spoke with as well as the date and time should there be any question later on about what you were quoted.

Unfortunately, I do NOT accept the following health insurance plans: Kaiser Permanente, Medicaid/Medicare, and Oregon Health Plan (OHP). I am considered OUT OF NETWORK with Providence and Blue Cross/ Blue Shield. What this means is that on some plans you will be covered, on others not. I advise clients with these plans to call ahead of time to the number on the back of your card and ask the following: 1. Do I have an out of network provision, and if so can I see Ms. Cogswell with this provision? 2. What, if any, is my deductible and 3. What is my copay? Often with out of network provisions there is a deductible, this means you will have to pay this out of pocket before your insurance starts covering. If you would like to see me with any of the above listed plans, I require payment in full at each session and will reimburse you promptly after your insurance reimburses me.

GOVERNMENT EMPLOYEES generally CAN see me as if I was an in network provider. If you work for State or Federal Government, you more than likely will be fully covered to see me. Again, your best option is to call your plan and ask, especially if you are local or County employed as this can sometimes vary.

EMPLOYEE ASSISTANCE PROGRAMS, or "EAP"
Most companies these days with more than 40 employees offer their employees this wonderful and totally free benefit called employee assistance programs. I am panelled with many EAP programs and if I'm not on yours, there's a good chance I can get on it very easily if you request that when you call the EAP. Although limited in sessions, generally 3 to 6 but sometimes more, this is often a great way to get started in therapy and with no copay it certainly is the most affordable option. Generally speaking you can start with your EAP and then transition over to your heath insurance if you need to continue beyond these sessions, as long as I am on your plan (see above). Ask your Human Resources person to see if your company offers EAP, you'll be glad you did.

Fees for Cash/Check paying Clients:
  • Individual therapy: $85.00/hr
  • Couples/Family therapy: $95.00/hr
Prepaid package of 4 sessions:
  • 4 individual = $300
  • 4 couples/family = $350
Terms for Prepaid packages:
  • 1. Clients may elect to purchase 4 sessions for the discounted prices (as listed above) at the end of the first session.
  • 2. There are generally no refunds for prepaid packages. Should there be extraneous circumstances, any refunds are provided solely at the discretion of Mimi Cogswell, LPC and will be based on the regular hourly rate of services already provided.
  • 3. Prepaid sessions are not transferable to other individuals or family members without prior consent by Mimi Cogswell, LPC.
